📋 Description

• Conduct 30-minute personalized 1-on-1 coaching sessions using motivational interviewing and evidence-based behavior change strategies • Surface intrinsic motivation and co-create actionable SMART goals • Facilitate group coaching sessions and workshops • Provide ongoing accountability, evaluate progress, and adapt strategies to support long-term member success • Administer appropriate screening tools and health measurements within scope of practice • Maintain timely clinical documentation and provide customized care plans • Serve as a behavior-change subject matter expert to the multidisciplinary care team • Use health conditions and trend data to guide members to appropriate care and eliminate barriers • Connect members with digital health tools, health plan benefits, employer wellness initiatives, and community resources • Resolve member concerns empathetically and professionally • Conduct proactive outreach calls to re-engage members and track participation • Manage scheduling, appointment coordination, and follow-ups for an active member panel • Participate in wellness campaigns, incentive programs, and client events • Escalate clinical or technical concerns according to operating protocols • Engage in quality assurance reviews, call audits, and routine feedback • Perform additional duties as assigned

🎯 Requirements

• National Board-Certified Health and Wellness Coach (NBC-HWC) credential required • Bachelor’s degree in Health Science, Kinesiology, Nutrition, Psychology, or a related field required • 2+ years of health coaching experience • Bilingual English/Spanish • Certified Diabetes Prevention Program (DPP) Lifestyle Coach preferred • Experience delivering care via telehealth or working within a remote team structure preferred • Strong baseline knowledge distinguishing health coaching, health education, and behavioral health/therapy boundaries • Proven customer service excellence and a track record of driving patient engagement and retention • Technical proficiency with Electronic Medical Record (EMR) systems, Salesforce, and Google Workspace preferred • Exceptional verbal and written communication skills • Strong caseload management, multitasking, and attention to detail • Ability to work in a fast-paced, evolving environment and accept continuous feedback • Willingness to adjust working hours based on member, company, and client availability needs • Must be able to work Monday–Friday, 8am–5pm PST • Maintain a dedicated, private, and professional workspace for virtual visits • Adherence to NBHWC Code of Ethics, HIPAA, PHI/PII protection protocols, and IT security standards
