📋 Description

• Care for stage 3 and 4 heart failure patients, ensuring medication compliance, dietary and lifestyle monitoring, regular follow-ups, and care coordination • Deliver evidence-based care to reduce avoidable hospitalizations and maximize quality of life • Collaborate with cardiologists, physician specialists, field-based nurses, community health workers, and clinical care teams • Train, mentor, and support local market APPs caring for cardiac patients in the home • Develop training on cardiac condition recognition, escalation criteria, medication management, patient education, care coordination, and documentation standards • Partner with cardiology leadership and local clinical teams to identify education needs and promote evidence-based protocols • Conduct patient assessments in homes and virtually • Counsel and educate patients and families about health programs and benefits • Document histories, examinations, medications, allergies, assessments, plans, and other required records • Complete documentation and paperwork promptly • Prescribe medications, order tests, and collaborate with cardiologists and physicians • Measure and report improved patient quality outcomes daily • Assist patients with educational video enrollment • Participate in integrated care team meetings • Develop, implement, follow up on, and evaluate treatment plans

🎯 Requirements

• Active and unrestricted Registered Nurse and Nurse Practitioner license in AZ, CA, or WA, or ability and willingness to obtain multiple state licenses • Board certified by ANCC or AANP • Current and unrestricted DEA certificate • Minimum of two (2) years of experience as an NP working in an inpatient or outpatient cardiology setting; no new graduates • Ability to work without direct supervision and practice autonomously • Access to transportation, valid driver's license, and car insurance • Proficiency with medical instruments and equipment • Knowledge of computer-based data management programs, information systems, medical records, and point-of-interview technology • Effective verbal and written communication with retail and medical partners, patients, family members, physicians, and community representatives • Understanding of federal and state regulations, including HIPAA and OSHA • Background in patient assessment, diagnosis, treatment, and management of cardiac diseases • Experience educating, coaching, or precepting APPs or clinical teams on cardiac disease management preferred
