📋 Description

• Provide In-Home Health Evaluations in members’ homes • Engage with people and help bridge gaps in care • Conduct approximately 45-minute visits, including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Build trusted relationships and help improve health outcomes • Visit people nationwide and connect them to appropriate care • Fulfill the requirements of the specified clinician role with Signify Health • Travel locally and within licensed states, with coverage across all counties as needed • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams and bone density screening

🎯 Requirements

• Nurse Practitioner or Physician Assistant • Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Able to travel locally and within licensed states • Willingness and ability to travel up to 100% of the time, including possible overnight hotel stays, car rentals and/or flights • Able to conduct virtual visits as needed
