📋 Description

• Provide In-Home Health Evaluations in members’ homes • Engage with people to help bridge gaps in care • Review medications and medical history • Conduct physical evaluations • Perform additional diagnostic tests when ordered, including diabetic eye exams, spirometry and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services products • Travel locally and across the defined coverage area and licensed states • Build trusted relationships and connect members with appropriate care • Visit people nationwide to support improved health outcomes

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Comfortable and able to safely perform duties in homes containing household pets, secondhand smoke and other potential airborne allergens or irritants • Able to travel locally and within licensed states • Nurse Practitioner or Physician Assistant qualification • Ability to work 40 hours per week • Ability to travel up to 100% of the time, including possible overnight hotel stays, car rentals and/or flights
