📋 Description

• Lead the in-home multi-specialty polychronic care model in an assigned market of 5–10 practices • Develop and oversee clinical strategies, policies, programs, processes, protocols, guidelines, and operations • Oversee daily clinical and business operations, including direct patient care, care management, social worker support, and pharmacy services • Review and approve APP, RN, social worker, and PharmD plans of care • Review, approve, and co-sign APP encounters • Ensure adherence to evidence-based clinical pathways and the Monogram Model of Care • Drive clinical outcomes, intervention closure, utilization management, prescribing and therapy management, and HEDIS/gap closure • Lead daily high-risk and concurrent review rounds • Supervise frontline clinical and operations team members • Delegate operational responsibilities to the Market Manager, including engagement, scheduling, administration, strategic implementations, and P&L management • Present market performance and outcomes to executive and senior leaders • Provide direct and indirect patient care, including disease diagnosis and treatment • Collaborate with patients, community physicians, facilities, and health plan partners • Conduct peer-to-peer reviews • Order labs and referrals and complete actions to improve outcomes and close care gaps • Guide population health management activities, including high-risk patient intervention and reducing admissions/readmissions • Conduct post-hospital discharge visits and patient reviews • Collaborate with Medical Economics, Finance, and other stakeholders on utilization trends • Participate in Monogram on-call activities, with at least 7 days on call once per quarter • Provide coverage for other MPEs during PTO or vacancies

🎯 Requirements

• Willingness and ability to obtain hospital privileges at required facilities • Demonstrated experience applying evidence-based clinical criteria • Experience in renal care and geriatrics • Strong management and communication skills • Active, unrestricted state medical license required in each state within the market • Experience with high-need Medicare Advantage and managed Medicaid populations • Experience with NCQA, HEDIS, Medicaid, Medicare, quality improvement, medical utilization management, and risk adjustment • Current unrestricted state medical license, with no Medicaid or Medicare sanctions • Willingness to become licensed in multiple states • MD or DO degree from an accredited medical school • Board certified or board eligible in an ACGME-approved specialty, such as Nephrology, Internal Medicine, Family Practice, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology, or Geriatrics
