📋 Description

• Serve as the clinical escalation point across the MSK care journey, guiding triage recommendations and resolving complex or unclear clinical situations • Conduct evidence-based clinical intake assessments and determine appropriate care pathways • Provide clinical guidance, education, and shared decision-making support • Build trusted patient relationships through high-touch telephonic engagement • Educate patients on Motion MSK programs and available care resources • Conduct outreach and proactive follow-up to initiate or adjust care pathways • Monitor patient progress and re-triage care plans when necessary • Use motivational interviewing to promote engagement, adherence, and persistence • Coordinate care among patients, caregivers, primary care providers, specialists, and affiliate providers • Oversee navigation pathways for patients escalating to surgical care • Collaborate with Care Coordinators, Outreach Specialists, and cross-functional partners • Manage high-volume inbound and outbound phone workflows • Document patient interactions and care navigation activities in CRM and EMR systems • Manage structured workflows supporting care coordination, clinical oversight, and patient experience • Perform other related duties supporting patient care needs, workflows, and business priorities

🎯 Requirements

• Doctor of Physical Therapy (DPT), Doctor of Physiotherapy, or Master of Physical Therapy (MPT) • Active, unrestricted Physical Therapist license in Oregon • Ability to obtain and maintain licensure in additional states and/or PT Compact privileges as needed • 5+ years of orthopedic or musculoskeletal clinical experience • Strong clinical judgment and triage skills • Experience in motivational interviewing, patient coaching, and supporting behavior change • Comfort working in a highly telephonic environment with inbound and outbound patient interactions • Excellent interpersonal and communication skills • Experience collaborating within multidisciplinary care teams • Ability to work within structured clinical workflows and guardrails • Comfort with EMR systems and remote clinical or care navigation tools • Access to a private, secure, HIPAA-compliant remote work environment • Must reside in the Pacific, Mountain, or Central timezone • Willingness to provide coverage across additional U.S. time zones as needed
