• Independently conduct Medication Therapy Management (MTM) and Stars Adherence initiatives within established protocols. • Conduct proactive patient outreach to improve engagement, adherence, and program participation. • Coordinate patient care activities to ensure timely medication access and continuity of therapy. • Prioritize work and resolve routine issues with minimal supervision. • Collaborate with Clinical Engagement Center team members to support consistent clinical services and shared accountability for outcomes. • Educate patients on program benefits and medication adherence. • Identify risks, address barriers, and determine appropriate interventions within program protocols. • Facilitate prescription renewals and extended day supply requests. • Manage exceptions and coordinate with pharmacies or providers as needed. • Connect members with appropriate clinical or operational resources. • Apply professional judgment and escalate situations requiring additional guidance. • Adhere to protocols and regulatory requirements, escalating complex, sensitive, or high-risk situations. • Complete assigned workflows accurately and efficiently. • Perform other duties as assigned.
• High School diploma or GED required. • Certified Pharmacy Technician certification required. • State Pharmacy Technician license required where applicable. • 3 years of pharmacy support experience or clinical program execution required. • Active national pharmacy technician certification through either PTCB or NHA is strongly preferred. • Fluency in both English and Spanish is strongly preferred. • Strong pharmaceutical knowledge required. • Demonstrated ability to deliver empathetic, patient-focused support and clear, respectful communication across diverse populations required. • Demonstrated experience managing inbound and outbound communications while following established workflows, scripts, and escalation protocols required. • Knowledge of healthcare regulations, including HIPAA, required. • Demonstrated ability to collaborate within cross-functional teams required. • Participate in, adhere to, and support compliance program objectives. • Ability to interact cooperatively and respectfully with other employees.
