📋 Description

• Conduct clinical assessments and provide disease- and therapy-specific education to patients and caregivers • Identify and address patient needs related to therapy use, adherence, persistence, adverse effects, and disease-state concerns • Triage concerns to a pharmacist and/or notify the treating provider when clinically appropriate • Document clinical assessments, patient interventions, care coordination activities, and clinical notes in patient records • Support the planning, implementation, and maintenance of clinical programs and projects • Complete adverse event forms and program-specific documentation • Collaborate with pharmacists, clinical team members, manufacturer and client partners, provider offices, and clinics • Assist with order creation and setup, payment methods, copay assistance, order details, and shipment coordination • Maintain clinical competency across treatments, disease states, protocols, and specialty pharmacy services • Ensure compliance with accreditation standards, pharmacy policies, HIPAA, and applicable regulations • Perform other duties as assigned

🎯 Requirements

• Associate Degree in Nursing (ADN) required • Current, unrestricted licensure as a Registered Nurse in the state of residence required • Ability to obtain additional single-state nursing licenses as needed • Unrestricted multi-state compact nursing license preferred • 2 years of patient-centered care or disease management nursing experience preferred • Participate in, adhere to, and support compliance program objectives • Ability to consistently interact cooperatively and respectfully with other employees • Ability to work remotely while residing in Arizona, Alabama, Arkansas, California, Colorado, Florida, Georgia, Illinois, Indiana, Iowa, Louisiana, Maryland, Michigan, Minnesota, Missouri, Nebraska, Nevada, New Jersey, New York, North Carolina, Ohio, Oklahoma, Oregon, Pennsylvania, Tennessee, Texas, Utah, Virginia, Washington, or Wisconsin

🏖️ Benefits

• STAR Bonus % (At Risk Maximum): 7.50% • Remote work • 5 x 4-hour shifts per week, Monday-Friday (typically 11:00 AM to 3:00 PM CT or 3:00 PM to 7:00 PM) • Ability to obtain additional single-state nursing licenses as needed
