• Provide emergency services to individuals on a rotating basis, including evenings, weekends, and holidays • Provide clinical care services to individuals with serious mental illness or emotional problems • Interview individuals to obtain clinical history for psychosocial assessments • Complete initial bio-psychosocial assessments • Assist with medication setup and “eyes on” medication compliance monitoring • Provide linkage, referral, and ongoing case management monitoring • Provide individual, group, and family therapy, psychosocial assessments, mental status exams, diagnosis, and care management • Provide or coordinate psychological, personality, and intelligence testing and integrate results into individual plans of service • Plan and administer therapeutic treatment and interventions • Consult with treatment teams and amend plans of service as recommended • Coordinate consultation, reporting, referrals, and services with agency staff, community organizations, and external healthcare professionals • Measure clinical outcomes and individual satisfaction • Perform clinical peer reviews of assessments resulting in denial of CMH services • Develop, implement, review, and update plans of service with individuals • Document contacts within 24 hours in individual records • Assist with financial assistance applications, including SSI, SSDI, Medicaid, GA, and MA • Conduct SSI level-of-care determinations and coordinate with the Social Security Administration • Conduct emergency psychiatric assessments and triage in person or by telephone • Develop individual safety plans and link individuals to services • Provide education to individuals, families, guardians, and staff • Maintain case files according to agency, regulatory, and accrediting requirements • Coordinate voluntary and involuntary hospitalizations and work with Probate Court • Advocate for individuals’ needs and rights • Comply with agency policies and procedures
• Minimum of a Master’s Degree in Social Work, Psychology or Counseling • Licensure in the State of Michigan as a Limited Licensed Master’s Social Worker, Licensed Master’s Social Worker, Temporary Limited Licensed Psychologist, Limited Licensed Psychologist, Temporary Licensed Professional Counselor or Licensed Professional Counselor • Must obtain full clinical licensure within 6 months of being eligible for such licensure • Must be at least 18 years old • Successfully attained a high school diploma or GED • Valid driver’s license • Must meet the state qualifications to become a QMHP, QIDP, and CMHP • Follow training requirements established by licensing agencies, accreditation bodies, MDHHS, and NorthCare Network upon hire, annually, and as necessary
