📋 Description

• Complete assigned Home Health QA reviews accurately, timely, and according to company standards, client expectations, ICD-10-CM guidelines, OASIS guidance, payer requirements, AI-supported workflow expectations, and internal quality protocols • Complete assigned reviews against quota and prioritize AI routing queues, holds, and urgent items within turnaround time • Validate ICD-10-CM coding accuracy, sequencing, specificity, and guideline compliance • Confirm OASIS responses are clinically supported, internally consistent, and aligned with CMS guidance • Cross-reference assessments, referrals, physician documentation, therapy notes, wound care, medications, and hospital records • Review high-risk items including GG items, functional status, M1800 ADLs, cognition, pain, dyspnea, falls, wounds, medication management, hospitalization risk, and discharge disposition • Place charts on hold or escalate incomplete, unsigned, unclear, or contradictory documentation • Write actionable QA comments • Review AI-generated coding, OASIS, and documentation suggestions using independent clinical judgment • Report recurring errors, missed cues, and workflow gaps through approved channels • Maintain HIPAA compliance and confidentiality across patient, client, and AI workflow data • Maintain tracking, hold notes, and certification paperwork • Maintain quality score and participate in secondary reviews, audits, training, and calibration sessions

🎯 Requirements

• Real home health chart review experience • Active or in-progress OASIS certification: HCS-O, COS-C, or another Nestmed-approved credential • Current, applicable ICD-10-CM coding credential, such as HCS-D, BCHH-C, CPC, or RHIT • Comfort working inside an AI-assisted workflow • Strong, concise writing for QA comments • Ability to escalate instead of guessing when documentation is unclear • HIPAA compliance and confidentiality • Ability to work effectively in an AI-enabled review environment
