• Review electronic health records to identify clients requiring timely follow-up • Identify care gaps and notify clinicians and supervisors of client follow-up needs • Monitor completion of outreach and follow-up activities until care gaps are resolved • Facilitate timely communication between providers and clients by ensuring appropriate outreach activities are completed • Track quality measures, care gap closure, follow-up activities, and performance metrics using Microsoft Excel and other reporting tools • Maintain tracking spreadsheets, databases, and reports, and prepare routine quality performance reports for leadership • Support adherence to agency quality standards, clinical protocols, payer requirements, and value-based care initiatives across mental health, substance use disorder, psychiatry, and medical services • Communicate with clinical staff and supervisors through email and other communication methods • Maintain confidentiality of protected health information in accordance with HIPAA and agency policies • Assist with quality improvement initiatives, departmental projects, and administrative duties
• High school diploma or equivalent • Minimum of one (1) year of experience in healthcare, behavioral health, medical office administration, quality support, or a related administrative position • Proficiency in Microsoft Excel, Outlook, and Microsoft Office applications • Experience working with electronic health records (EHRs) • Experience in behavioral health, healthcare quality, population health, or value-based care • Exceptional attention to detail and organizational skills • Ability to perform structured record review accurately and consistently • Intermediate Microsoft Excel proficiency, including maintaining tracking spreadsheets, sorting and filtering data, and preparing basic reports • Strong written communication skills, particularly professional email communication • Persistent follow-through and ability to monitor tasks through completion • Ability to prioritize multiple responsibilities while meeting deadlines • Valid Ohio Driver’s License with a safe driving record and valid insurance • Successful completion of pre-employment screenings, including drug tests, background checks, physical examination, and TB test • Commitment to maintaining strict confidentiality and safeguarding sensitive client information
