📋 Description

• Provide expert clinical guidance by phone to patients experiencing emergent symptoms • Assess patient situations, ask appropriate questions, and determine the appropriate level of care • Manage a high volume of calls and MyChart messages • Document and communicate patient needs using CareConnect • Provide nursing care to patients and families across the health illness continuum • Collaborate with the healthcare team and represent primary care providers professionally and empathetically • Evaluate and document patient responses to nursing interventions and achievement of outcomes • Revise plans of care based on evaluative data as part of a multidisciplinary team • Delegate nursing tasks to licensed practical nurses and unlicensed assistive personnel according to the Ohio Nurse Practice Act • Participate in process improvement activities targeting clinical quality, customer satisfaction, and financial performance • Perform duties with focus on safety, quality, and efficiency as part of a High Reliability Organization

🎯 Requirements

• Associate's Degree in Nursing (Required) • RN — Registered Nurse — Ohio Board of Nursing • BTLS — Basic Trauma Life Support — American Heart Association • CPR — Cardiopulmonary Resuscitation — American Red Cross • 5 years nursing experience • Prefer 2 years’ experience in a Primary Care setting or similar ambulatory care setting • Strong multitasking skills • Excellent listening abilities • Clinical assessment and sound clinical judgment • Ability to manage a high volume of calls and MyChart messages • Ability to document and communicate patient needs in CareConnect • Must work from Ohio
