📋 Description
• Deliver targeted, organized, coordinated, cost-effective, and patient-centered care • Partner with patients, families, caregivers, and providers • Collaborate with interdisciplinary staff and stakeholders across the care continuum • Promote patients’ optimal health through disease prevention and management strategies • Apply evidence-based interventions to patient populations • Support disease-based quality measures, patient satisfaction, access to services, and hospital/ED utilization outcomes
🎯 Requirements
• Bachelor's Degree • APRN - Advanced Practice Registered Nurse License - State of Ohio Board of Nursing • BLS - Basic Life Support - American Heart Association • RN - Registered Nurse - Ohio Board of Nursing • Ability to work 40 scheduled hours weekly on the day shift
