📋 Description

• Coordinate and provide intermediate direct patient care in patients’ homes or alternate infusion suites under a physician’s plan of care • Initiate, develop, implement, and evaluate intermediate nursing plans of care • Perform treatments, administer medications, and educate patients and families/caregivers • Modify treatment plans in response to changing patient status or physician orders • Assess patient needs and physical status during skilled visits • Obtain and clarify physician orders and promptly inform physicians of significant patient-condition changes • Provide training and mentorship to Infusion Nurse I staff as needed • Teach patients and families to independently manage prescribed therapy and care needs • Communicate with clinical teams, medical providers, patients, and caregivers to ensure continuity of care • Prepare and submit real-time clinical documentation • Coordinate discharge planning and prepare discharge summaries • Comply with agency policies, professional standards, regulations, and accreditation standards • Use computers and automation technologies to validate orders, document care, and follow assigned visit schedules • Respond to patient requests and initiate emergency procedures when necessary • Safeguard confidential patient and proprietary information in accordance with HIPAA • Explain Option Care services and related financial obligations to patients • Complete required training and continuing education to maintain competency and licensure • Provide oversight and coordination of paraprofessionals and act as Supervisor when applicable • Participate in multidisciplinary conferences, precepting, training, mentoring, and nursing on-call responsibilities • Insert PICC/Midlines and troubleshoot or declot obstructed central-line catheters when applicable • Monitor PICC/Midlines and educate patients on infection prevention and routine care

🎯 Requirements

• Active and unrestricted RN license in the state of practice • Minimum of 1 years of nursing experience • Current CPR certification required • Demonstrated competency in patient care standards required for safe delivery of services and infusion skill sets applicable to agency programs and service needs • Advanced certification and training as applicable • Required licensure to operate a motor vehicle in the state of practice • Access to a vehicle for business travel with proof of liability insurance • Basic knowledge of computer operating systems and software applications • Ability to lift up to 50 pounds with a maximum lifting of 75 pounds • Willing to travel up to 100% of the time for business purposes • Bachelor of Science in Nursing as granted by an accredited school of nursing preferred • 1-2 years of previous infusion nursing experience preferred
