📋 Description

• Guide, mentor, teach, and support individuals, elderly participants, and individuals with disabilities using Personal Options services • Meet with participants in their homes at least every six months • Demonstrate expertise in Personal Options services, West Virginia waiver programs, and Palco policies • Educate participants and designated representatives about program rights, responsibilities, and risks • Monitor and evaluate participants’ ability to participate in the program • Assist with developing and updating service/spending plans • Monitor service/spending plan utilization • Respond to member and representative complaints and grievances • Act as a mandatory reporter for suspected abuse, neglect, exploitation, and other required incidents • Use a person-centered approach in participant interactions • Document participant visits accurately on a company-issued laptop • Log events and run reports electronically • Perform daily administrative tasks, including scheduling visits and responding to emails and calls • Follow Palco operational procedures • Participate in shadowing and periodic training • Distribute new information to assigned participants • Safeguard company equipment and data in accordance with company policies and HIPAA privacy practices

🎯 Requirements

• A minimum of three years of experience working with the public is required • Experience as a patient navigator, community connector, community health worker, lay health advisor, health educator, or case manager preferred • High School Diploma or equivalent required • Professional verbal and written communication skills • Ability to electronically document visits • Proficient skills with Microsoft Office, Microsoft Outlook, and Internet • Good problem-solving skills • Willingness to travel within a geographically established region to meet participants in their homes • Willingness to work with people of all ages and disabilities • Valid driver’s license and dependable mode of transportation • Active prepaid or contract cell phone plan for continual communication • Ability to pass a criminal background check and maltreatment check • Travel is required
