PAA targets value-based and risk-based contracts (bundles, ACOs, Medicare Advantage) and positions its solutions to reduce readmissions, lower total cost of care, and improve in-network utilization, delivered as a SaaS/cloud solution with integrations and partner programs.

📋 Description

• Monitor engagement for assigned clients on a daily and weekly cadence using engagement dashboards and reports • Track documents, head-in-bed, signed NOMNCs, and conversation responses against goals • Build outreach lists for facilities with no active user or users below goal • Make outbound calls from a personal cell phone, including cold calls, to find the right contact and register them in Anna • Conduct templated and mass email campaigns to below-goal facilities • Onboard and train new PAC users on Anna workflows • Coordinate with the Training team on onboarding and consistent messaging • Validate engagement data, including confirming document uploads completed through integration • Keep facility records and outreach notes current • Measure whether outreach moved engagement metrics • Escalate integration gaps to Network Operations • Escalate repeated non-compliance or client-side issues to Client Success Executives • Capture PAC user feedback for Product • Prepare engagement reporting for leadership and clients, including scorecards, PAC facility snapshots, and monthly reporting • Contribute to network-wide communications such as newsletters • Work closely with Network Operations, Client Success Executives, and the Training team • Represent PAC users internally

🎯 Requirements

• Three or more years of experience in provider relations, provider outreach, network management, or care coordination within a healthcare organization, health plan, or managed care setting • Strong familiarity with post-acute care providers, including SNFs, LTACs, home health, hospice, and inpatient rehab • Understanding of how post-acute care providers work with health plans on reviews, authorizations, and discharges • Proven experience reaching providers by phone and email, including cold outreach, and building working relationships that improve performance • Comfort making regular outbound and cold calls • Ability to manage a large facility list and prioritize from data • Organized and consistent record-keeping • Clear, patient communication with facility staff at all levels • Proficiency in Microsoft Excel, including filters, pivot tables, and lookups, and Outlook • Bachelor’s degree required • Experience using dashboards and reporting tools such as Tableau or Power BI and spreadsheets to spot trends and prioritize work • Familiarity with NOMNCs, CMS regulations, Medicare Advantage, and Medicaid requirements related to post-acute care • Experience with a CRM • Quiet, private workspace suitable for phone calls and handling PHI • Reliable internet connection • Ability to handle extended periods of talking on the phone and working at a computer • Adherence to HIPAA and PAA’s information security and privacy policies
