📋 Description

• Initiate phone contact with discharged patients within the CMS-required timeframe • Conduct patient experience and recovery queries and provide follow-up to patients and family members • Review discharge summaries, medication lists, and follow-up instructions • Perform medication reconciliation and coordination support • Assess understanding of discharge plans, symptom management, and medication adherence • Document patient responses, advice, outreach attempts, interactions, and outcomes in the EHR • Identify potential complications and escalate concerns to providers or the care team • Schedule primary care appointments and coordinate availability with front desk and clinical teams • Reinforce discharge education, including when to contact a provider or seek emergency care • Support readmission prevention and address barriers to care • Review and address open care gaps, including hypertension, diabetes monitoring, preventive screenings, and wellness visits • Collaborate with providers, care coordinators, and case management teams • Participate in team discussions and quality improvement initiatives • Ensure compliance with documentation and billing requirements • Provide hypertension-focused education and facilitate follow-up for elevated blood pressure or medication concerns • Collect patient data, complete required forms, identify problems and priorities, and refer issues requiring follow-up • Develop and evaluate follow-up care plans using the nursing process with patient and family involvement • Perform other duties as assigned

🎯 Requirements

• Associate degree in Nursing required • Bachelor's degree in Nursing preferred • One (1) year experience as a registered nurse • Current RN compact/multistate license recognized by the NCSBN Compact State, or license to practice as an RN in the state where working • RN diploma or certificate accepted in lieu of an associate degree in nursing with the required current RN license • Knowledge of discharge care, medication reconciliation, patient follow-up, symptom management, and medication adherence • Ability to identify complications, escalate concerns, and coordinate follow-up care • Ability to document in the EHR according to Prisma Health and CMS requirements • Critical thinking and advanced nursing observation skills • Understanding of patient rights, confidentiality, ethical issues, and age-related patient needs
