📋 Description

• Assess clients and develop person-centered care plans • Update care plans as client conditions change • Conduct supervisory visits to evaluate caregiver performance and care plan adherence • Train and support caregivers • Document all assessments, updates, and visits • Communicate with clients, families, and staff to coordinate care • Ensure compliance with regulations and standards • Participate in quality assurance • Oversee care plans and support caregiver supervision to ensure a high standard of care

🎯 Requirements

• Current and active Registered Nurse (RN) license in { job.state } • Minimum of 1–2 years of nursing experience (home care or geriatric care preferred) • Strong assessment and documentation skills • Excellent communication and interpersonal skills • Ability to supervise and support non-clinical staff • Detail-oriented with strong organizational and time management skills • Valid driver’s license and reliable transportation for home visits • Familiarity with state home care regulations and best practices
