📋 Description

• Provide support to the Care Management team that coordinates care • Work closely as a healthcare team member • Perform complex tasks related to overall team functions and activities identified during initial and ongoing assessment and management of patients • Support patient-focused, whole-person care under the supervision of the Regional Director of Care Management

🎯 Requirements

• 5 years Hospital, clinical setting or healthcare related field • 3 years demonstrated outstanding customer service experience • 3 years clerical and computer experience • 3 years knowledge of insurance billing and managed care • Some college-level coursework or high school diploma/GED equivalent preferred, particularly Medical Terminology or Medical Assistance coursework • CHAA preferred • CHAA required within 6 months (180 days) of hire if in California • 3 years EPIC Case Management and ADT Patient Access preferred
