📋 Description

• Support integrated primary care through prevention and wellness strategies. • Manage and coordinate care for patient panels. • Monitor patient attribution and identify gaps in care. • Coordinate preventative services and wellness screenings. • Conduct outreach to patients and health care providers. • Connect patients with appropriate services, care team members, and resources. • Support the clinic care team in managing preventative screenings and wellness strategies. • Maintain the integrity of the EMR and manage chronic disease registries, provider scorecards, and result reconciliation. • Promote quality initiatives and improved patient outcomes. • Track and report quality measures and validate patient record accuracy. • Collect subjective and objective patient or caregiver data to identify barriers to care and refer to care management roles. • Communicate collected data, obtain appropriate orders, and follow through on care plans under care team direction.

🎯 Requirements

• Post secondary education of one - two years, with an emphasis in healthcare or medical related field is preferred. • A minimum of one year work experience in ambulatory care, community outreach, patient or provider support, customer service, and/or various integrated care systems preferred. • Training in Motivational Interviewing and/or Mental Health First Aid preferred. • Background as a Certified Medical Assistant (CMA) is required. • Nationally certified or registered as a Medical Assistant (MA) required. • Demonstrates computer literacy including using Microsoft Office products and electronic health record (EHR). • High level of computer skills and ability to run registries. • Customer service, critical thinking, multi-tasking, medical and medication terminology, and medical paperwork knowledge including insurance completion/submission requirements.
