📋 Description

• Reconcile Emergency Department, Urgent Care, and/or Observation logs to patient charts to initiate coding and billing. • Produce a minimum of 85-100 charts daily through manual pull or processing of stray and deficient charts. • Acquire logs and charts through EMR, SFTP, DFS, scanning, and feed sources. • Format logs and charts into required Excel or other formats. • Analyze charts for required documents, complete provider documentation, and matching date of service, account number, and MRN information. • Submit chart batches via Chart Receiver and confirm receipt in Kofax, ensuring correct properties, legibility, orientation, page limits, and removal of unnecessary pages. • Identify and assign deficient and non-bill charts using approved reasons. • Research and resolve outstanding strays, EDRC exceptions, and deficient charts at least twice weekly or as reports are returned. • Acquire, reconcile, analyze, and submit inpatient logs and charts monthly via Kofax by the 10th of the month. • Submit logs and charts within 3-4 days of date of service or facility-specific deadlines using Chart Portal and Chart Receiver. • Assist with offshore audits, non-bill audits, and other projects. • Communicate with providers regarding incomplete charts through in-person, EMR, telephone, My SCP App, or email. • Meet departmental chart submission lag-time and monthly deadline goals. • Report daily production data in JotForm. • Maintain HIPAA and SCP Health compliance; report concerns to the Manager or Compliance Team. • Collect SCP Health mail at the facility and send it to the Dallas office weekly if HI-C onsite. • Function as an onsite SCP Health representative for providers and hospital staff. • Perform additional assigned duties.

🎯 Requirements

• High school diploma. • 0-2 years of experience in Medical Records. • Experience in Medical Records, HIM, Healthcare Clinic or Physician Office preferred but not required. • Detail oriented, with ability to organize tasks and set priorities. • Basic knowledge of Microsoft Office, including Microsoft Excel, Microsoft Word, and Microsoft Outlook. • Professional attitude and appearance to communicate with hospital staff and providers. • Ability to maintain compliance with SCP Health Compliance policies and HIPAA guidelines. • Ability to work cross-trained at 1 or 2 other sites. • Ability to work onsite where applicable and comply with hospital policy.
