• Coordinate provision of grant-funded clinically integrated primary and secondary prevention services • Oversee integration of screening, patient education, and lifestyle counseling into the outpatient healthcare system • Oversee grant expenses and recommend measures for fiscal responsibility across federal, state, and/or tribal funding sources • Oversee patient screening, education, and follow-up data entry into Cerner and other registries/tracking packages • Conduct quality assurance reviews with Community Wellness staff, clinic managers, and medical executive committee members • Provide oversight of staff delivering health education and case management for patients due for screening and diagnostic services • Oversee patient tracking and follow-up, and coordinate quality delivery of treatment services, lifestyle interventions, and additional follow-up • Collaborate with SEARHC departments and strategic partners to improve access to comprehensive prevention and healthcare services • Coordinate program activities, service documentation, eligibility screening, grant-funded reimbursement, and service expansion • Collaborate with local, state, and federal agencies • Work with the Director of Community Wellness, Grant Writers, and Finance team on annual reports, continuation and competitive applications, and service indicator reporting • Hire, train, evaluate, and discipline subordinates
• Master of Public Health or other professional health degree; a Bachelor’s degree plus 2 additional years of related experience can be exchanged for a Master’s degree • 3 years’ experience in healthcare, public health, nutrition, or related fields, including grant coordination or management, health education, motivational interviewing, or lifestyle counseling services in a public health or ambulatory care setting • Experience working within complex systems of patient care and referral • Basic Life Support • 2 years supervisory experience • Advanced knowledge of cardiovascular health, breast, cervical, and colorectal cancer prevention and screening, diabetes prevention and control, tobacco control and cessation, physical activity promotion, and nutrition • Knowledge of health education core competencies, stages of change theory, motivational interviewing, and the socioecological model • Working knowledge of group dynamics and ability to facilitate/train groups • Good customer service skills • Advanced computer application skills including email, word processing, spreadsheet, database, and document organization • Excellent oral/written communication and interpersonal skills in relating to patients and staff sensitively and culturally competently • Superb organizational and prioritization skills • Problem analysis and solution skills in complex healthcare settings using a systems approach • Proficiency in SEARHC’s electronic health data system preferred • Ability to provide guidance and leadership to clinic staff and integrate service processes into existing clinic protocols • Ability to collaborate with other programs to maximize service delivery and avoid duplication of services • Required travel to grantor and partner meetings and site and supervisory visits
