• Provide integrated support to patients and physicians to improve patient care and medication adherence • Develop personalized relationships with patients and prescribers in assigned clinics • Provide medication management, reconciliation, dose optimization, and adherence counseling • Conduct phone, virtual, and video-based disease-state assessments and drug-therapy education • Identify, resolve, and prevent medication-related problems • Develop individualized care plans with interdisciplinary care teams • Monitor medication regimens for therapeutic goals, adverse effects, drug interactions, and clinical criteria • Recommend drug-therapy optimization and medication cost-saving initiatives • Participate in quality-improvement initiatives, clinical outcome studies, REMS compliance/reporting, and process-improvement projects • Provide clinical justification, literature, and rationale for prior authorizations and appeals • Collaborate with physicians, specialists, onsite clinic pharmacists, healthcare providers, departments, and stakeholders • Support occasional peer-to-peer requests • Perform targeted drug-level clinical assessments and educational counseling • Ensure clinical compliance with FDA guidance, clinical guidelines, and evidence-based literature • Optimize workflows and implement strategies to improve efficiency, productivity, and quality of care • Promote patient safety and medication literacy through education and counseling • Maintain confidentiality of organizational information • Attend team meetings, conferences, and professional-development activities • Stay current with therapy-management advancements and incorporate them into practice • Perform other duties assigned by the Senior Director of Clinical Services and Manager of Clinical Services
• Must hold a Doctor of Pharmacy (PharmD) • Must be a licensed pharmacist in California • Must be an advanced practice pharmacist in California • Must meet two of three advanced practice pharmacist criteria: current relevant certification, completed U.S. postgraduate residency, or 1,500 hours of clinical experience under a collaborative practice agreement/protocol within 10 years of application • Strong knowledge of pharmacology developments and FDA health regulations • Strong technical and clinical skills • Knowledge of disease-state management and treatment guidelines • Ability to analyze information and exercise good judgment • Strong verbal communication skills • Ability to manage processes and maintain legal compliance • Microsoft Office proficiency, including Outlook, Teams, Word, Excel, and PowerPoint • Ability to navigate internet and web-based platforms • Data entry and computer skills will be evaluated during pre-employment • Organized, dependable, empathetic, detail-oriented, quality-focused, and highly motivated • Discretion and confidentiality are essential when handling sensitive private data
