📋 Description

• Learn Solace’s proprietary system, tools, technology, and expectations • Build strong, trusting relationships with patients through listening and empathy • Identify and prioritize patients’ needs • Develop comprehensive patient care plans • Address patients’ social determinants of health, including food resources, transportation access, and support at home • Guide patients through complex healthcare concerns and help them achieve health and wellness goals • Navigate resources and resolve patients’ administrative and care-coordination problems via video or phone

🎯 Requirements

• Registered Nurse (RN) license in good standing • Experience working with diverse patient populations • Clinical and organizational skills to balance multiple tasks and work under pressure • Ability to learn new systems quickly and efficiently • Applicants must be based in the United States • Must be authorized to work in the United States • Some weekly availability during core business hours Monday–Friday • Prior care plan creation experience is not required • Comfortable supporting patients in a non-clinical capacity without providing direct patient care or medical advice
