📋 Description

• Conduct virtual adult and/or pediatric wellness visits, health assessments, and related healthcare services • Review caseload and prepare technology for patient service • Navigate the daily schedule using technology • Partner with clinical in-home team members visiting patients’ homes • Build welcoming connections with diverse patients • Collect relevant patient information and perform services • Conduct health risk assessments, medication reviews, and cognitive screenings • Provide patients with educational information about their health and well-being • Collaborate with Sprinters to evaluate vital signs and arrange blood draws • Carry out tasks addressing patients’ specific needs • Provide care coordination and management, including preventive care interventions, medication management, specialist referrals, community resources, and documentation of findings

🎯 Requirements

• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner license in Louisiana • Prior experience performing wellness visits • Professionalism, integrity, accountability, confidentiality, care, and compassion • Willingness to perform last-minute problem solving and out-of-the-box thinking • Technologically savvy and comfortable using laptops or mobile devices for charting • Comfortable using HIPAA secure messaging apps for care coordination • Strong written and verbal communication skills • Ability to work independently or in a team environment • Must pass national background check and valid clinical license search • Must complete required application questions regarding state licensure, wellness-visit experience, Medicaid enrollment, virtual Family Nurse Practitioner experience, notice period, prescriptive authority, and board certification
