Their full-stack platform and operations close care gaps, deliver preventive screenings and diagnostics, coordinate medications and referrals, and scale longitudinal primary care and wellness visits for payer and provider partners and patients.

📋 Description

• Conduct virtual adult and/or pediatric wellness visits, health assessments, and related services • Review caseload and prepare technology for patient care • Navigate the daily schedule and partner with clinical in-home team members visiting patients’ homes • Build welcoming connections with diverse patients and collect relevant information • Conduct health risk assessments, medication reviews, cognitive screenings, and patient education • Work with Sprinters to evaluate vital signs, arrange blood draws, and address patient needs • Provide care coordination and management, including preventive care interventions, medication management, specialist referrals, community resources, and documentation

🎯 Requirements

• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner License in Georgia • Prior experience performing wellness visits • Professionalism, integrity, accountability, confidentiality, care, and compassion • Willingness to perform last-minute problem solving and out-of-the-box thinking • Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA-secure messaging apps • Strong written and verbal communication skills • Ability to work independently or in a team environment • Pass national background check and valid clinical license search • Virtual experience as a Family Nurse Practitioner is assessed in the application • Experience with Medicare and/or Medicaid Annual Wellness Visits is assessed in the application • Current prescriptive authority in the state(s) where licensed to practice as a Nurse Practitioner is assessed in the application
