📋 Description

• Review the daily caseload and prepare technology for patient services • Navigate the daily schedule using modern technology • Partner with clinical in-home team members who visit patients’ homes • Connect with diverse patients and collect relevant information • Conduct virtual wellness visits, health risk assessments, medication reviews, cognitive screenings, and patient education • Evaluate vital signs with Sprinters and arrange blood draws • Provide care coordination and management, including preventive care, medication management, specialist referrals, community resources, and documentation • Deliver personalized, high-quality healthcare services to patients in their homes

🎯 Requirements

• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner License in Texas, Louisiana, or Oklahoma • Prior experience performing wellness visits • Professionalism, integrity, accountability, confidentiality, care, and compassion • Willingness to perform last-minute problem solving and out-of-the-box thinking • Technologically savvy with laptops, mobile devices, and HIPAA-secure messaging apps • Strong written and verbal communication skills • Ability to work independently or in a team environment • Pass national background check and valid clinical license search • Ability to conduct virtual adult and/or pediatric wellness visits • Relevant Medicare and/or Medicaid Annual Wellness Visit experience requested in the application • Virtual Family Nurse Practitioner experience requested in the application • Current prescriptive authority in the state(s) where licensed, as requested in the application
