• Schedule home health and hospice appointments as ordered by physicians • Schedule starts of care, initial assessments, resumptions of care, and routine visits • Schedule multiple disciplines across 3 counties and 10 territories throughout Central Oregon • Coordinate visits between disciplines by analyzing territory and caseload for efficiency and regulatory compliance • Enter appointments accurately into the scheduling system and communicate pertinent information to appropriate staff • Support Intake and triage by managing faxes, inbasket, email, referrals, labs, add orders, information requests, and documentation scanning • Communicate staffing changes, identify issues, implement countermeasures, and communicate changes to the patient care team • Monitor Home Health patients with inpatient status and coordinate transfers and timely resumption of care after discharge • Provide customer service and follow through to resolution or referral • Navigate EMRs and platforms including Teams, Excel, OneNote, Company Mileage, and RightFax; run and update department data reports • Screen for potential safety concerns and communicate them to appropriate personnel • Collaborate with the scheduling team, Intake team, patients, families, clinicians, leadership, and other patient care team members • Maintain professionalism, confidentiality, regulatory compliance, ethical conduct, and organizational integrity • Support the organization’s vision, mission, values, and Lean continuous-improvement principles • Perform additional duties of similar complexity as assigned
• High school diploma or GED • Previous customer service experience in a high volume, fast paced environment • Previous experience in an administrative or clerical position • Knowledge of medical terminology and procedures • Two years of general office experience, preferably in a hospital environment • Phone skills, computer experience, and interpersonal skills • Ability to wear appropriate Personal Protective Equipment (PPE) • Ability to use clear and audible speaking voice and hear normal speech levels • Ability to stand, sit, walk, lift 1–10 lbs, and operate a keyboard • Ability to bend, climb stairs, reach overhead, carry 1–10 lbs, push/pull 1–10 lbs, and grasp/squeeze • Ability to stoop/kneel/crouch, lift 11–25 lbs, carry 11–25 lbs, push/pull 11–25 lbs, and operate a motor vehicle • Preferred: Associate degree or higher; additional college courses in information systems, office management, and medical technology • Preferred: 1–2 years of scheduling experience in a fast-paced environment
