📋 Description

• Implement individualized physical therapy treatment plans as established and directed by the supervising Physical Therapist • Deliver therapeutic interventions including therapeutic exercise, gait training, balance activities, transfer training, and modality application • Monitor and document patient progress, functional status, and response to treatment in compliance with state, federal, and facility standards • Communicate regularly with the supervising PT, interdisciplinary team, and nursing staff regarding resident goals and progress • Educate patients, families, and caregivers on home exercise programs, fall prevention, and safe mobility techniques • Participate in care planning meetings, utilization reviews, and MDS documentation as applicable • Maintain timely, accurate clinical documentation in compliance with Medicare/Medicaid and facility requirements • Uphold resident rights, dignity, and confidentiality in all interactions • Assist in maintaining a clean, organized, and safe therapy gym and treatment environment

🎯 Requirements

• Associate's degree from an accredited PTA program • Current, unrestricted PTA licensure in the State of Colorado (or ability to obtain prior to start date) • Current CPR/BLS certification • Strong interpersonal and communication skills with a patient-first mindset • 1+ years of experience in a skilled nursing, long-term care, or post-acute rehabilitation setting (preferred) • Familiarity with MDS/PDPM documentation frameworks (preferred) • Experience with geriatric populations and fall prevention programs (preferred) • Knowledge of EMR/documentation systems used in SNF environments (preferred)
