📋 Description

• Care for patients throughout Colorado via telehealth • Evaluate, treat, and manage patients through individualized hormone optimization • Provide medical weight management and preventive healthcare • Maintain an approximately 50/50 patient population focus on men's and women's hormone replacement therapy • Deliver personalized care designed to improve patients' health, wellness, and quality of life • Treat patients using SynergenX clinical protocols and telemedicine systems • Provide telemedicine services to patients located throughout Colorado • Work Monday through Friday with patients scheduled between 8:00 AM and 5:00 PM Mountain Time • Complete seven days of paid remote training • Collaborate with supervising/collaborating physicians as applicable • Support a growing patient volume and potential additional hours

🎯 Requirements

• Active Colorado Family Nurse Practitioner (FNP) license with Colorado APRN licensure (Family specialty only), or an active Colorado Physician Assistant license under the primary taxonomy NPI • Current Colorado DEA registration required at the time of application; company does not reimburse DEA registration costs • Current National Provider Identifier (NPI), without specialty focus • Current board certification applicable to the profession, with documentation • Fully active and unencumbered Colorado advanced provider license • No current or pending malpractice claims or disciplinary actions • Strong clinical assessment and medical decision-making skills • Excellent patient communication and relationship-building abilities • Active DEA registration in Colorado or ability to transfer from the current state • Experience or strong interest in hormone replacement therapy, preventive medicine, men's health, women's health, and medical weight management preferred • Bilingual English/Spanish is a plus • Availability to begin employment with minimal delay and complete hiring and credentialing promptly • Candidates must disclose NPI number and board certification information during the application process • Candidates must be eligible to work in the United States as applicable
