📋 Description

• Support patients with complex behavioral health needs, including those moving between levels of care • Support outpatient psychiatry and therapy programs • Work with patients, clinicians, internal program teams, insurers, and external partners to remove barriers and coordinate services • Support patient enrollment, ongoing engagement, discharge, step-down, and return to outpatient care • Conduct timely outreach and help patients navigate benefits, transportation, community resources, specialty services, and other supports • Coordinate with psychiatrists, psychiatric mental health nurse practitioners, therapists, primary care providers, and external treatment partners • Identify changes in patient risk, engagement, or functioning and escalate clinical concerns according to established protocols • Facilitate referrals to emergency, inpatient, residential, or other higher-acuity services under clinical direction • Manage inbound and outbound referrals, transfer necessary records, and confirm completed handoffs • Document case management activity and patient communication in the electronic health record • Maintain internal program and external resource knowledge and improve service repositories and referral pathways • Track workload, referral status, barriers, outcomes, and care transitions to identify operational gaps • Build and refine case management workflows as Talkiatry expands complex-care services

🎯 Requirements

• Bachelor's degree in social work, psychology, counseling, public health, human services, or a related field • At least three years of behavioral health case management, care coordination, discharge planning, or healthcare navigation experience • Direct experience supporting higher-acuity behavioral health populations, including crisis care, discharge planning, structured treatment settings, or transitions between levels of care • Ability to work independently, manage multiple active cases, prioritize time-sensitive needs, and follow work through to completion • Strong judgment about when a situation requires escalation to a clinical review or immediate emergency workflows • Experience working with multidisciplinary clinical teams and external healthcare or community partners • Clear written and verbal communication skills • Experience documenting work in an electronic health record or care management platform • Experience using AI tools to increase efficiency • Comfort working in a virtual environment and adapting as programs, workflows, and responsibilities evolve • Licensure is not required; relevant credentials may be considered • Must be authorized to work in the U.S.; visa sponsorship or visa transfer requirements are addressed in the application
