📋 Description

• Lead case management initiatives that enhance patient outcomes and streamline care delivery. • Develop and implement strategies to reduce hospital readmissions and improve quality metrics. • Collaborate with cross-functional teams to ensure seamless coordination of care plans. • Analyze data trends to identify opportunities for process improvement and cost efficiency. • Mentor and support nursing staff to foster professional growth and excellence in care.

🎯 Requirements

• Current RN license in good standing; license must be in state of primary residence. • Minimum 2 years of clinical nursing experience, including 2 years in case management, care coordination, or prior authorization. • Strong analytical skills with ability to interpret healthcare data and apply insights. • Excellent communication and leadership abilities. • Reliable cable broadband or fiber optic home internet with at least 10Mbps download/5Mbps upload when working from home. • Bachelor's degree in Nursing preferred. • Experience with value-based care models and population health strategies preferred. • Proficiency in case management software and electronic health records (EHR) preferred.
