📋 Description

• Create a welcoming start for new program referrals by explaining services and setting clear expectations • Educate patients about their care and available resources • Serve as the primary clinical contact for patients • Coordinate with pharmacists, providers, and other care partners • Identify barriers, resolve concerns, and escalate complex issues • Support care coordination and utilization management processes • Document patient and provider interactions accurately • Identify trends and share insights to improve team performance and patient experience • Manage inbound and outbound calls in a fast-paced environment • Build expertise, welcome feedback, and contribute improvement ideas

🎯 Requirements

• Current, unencumbered Compact Registered Nurse license in your state of residence • At least 5 years of clinical experience in infusion, acute care, home care, specialty pharmacy, emergency care, or critical care • Strong clinical judgment, critical thinking, organization, and time-management skills • Clear verbal and written communication skills • Comfort handling inbound and outbound calls • Ability to work independently, manage several priorities, and respond calmly to changing needs • Proficiency with Windows-based programs, including Microsoft Word, Excel, Outlook, and PowerPoint • Preferred: Experience with Medicare, Medicaid, prior authorizations, or utilization management • Preferred: Bachelor’s degree in nursing or a related field • Home internet connection through cable broadband or fiber optic service with at least 10Mbps download and 5Mbps upload
