📋 Description

• Provide specialized sensory-motor, cognitive, communicative, social interaction, and behavioral training to promote skill development in individuals with developmental disabilities • Enable consumers to acquire knowledge and skills and participate as valued members of their communities based on their choices • Provide training to increase or maintain self-help, socialization, and adaptive skills • Support consumers in achieving and maintaining quality of life goals • Provide first aid and appropriate attention to injury and illness • Carry out programs identified in the consumer’s ISP and/or routine plan of care • Observe consumer behavior and immediately report potentially dangerous situations to the responsible person and supervisor • Schedule services and appointments according to family preferences and routines • Maintain accurate and timely required documentation • Implement ISP-approved attendant care plans • Assist with personal cleanliness and activities of daily living, including bathing, oral hygiene, toileting, dressing, ambulation, transfers, eating, meal preparation, and routine nail and skin care • Plan, shop for, store, and cook nutritional meals • Ensure consumers receive medications as prescribed • Maintain safe and sanitary living conditions, including cleaning, dishwashing, laundry, linens, household shopping, and garbage removal • Provide respite relief and supervision in the home or community • Support consumers’ emotional, physical, and mental well-being • Follow routine plans of care and provide dietary support • Assist consumers with transportation for daily living activities • Represent Family Partners, LLC professionally • Provide safe transportation to and from home and community activities

🎯 Requirements

• Must be 18 years of age or older • Must be able to obtain Level 1 Fingerprint Clearance Card • Ability to work weekends and evenings as required by the needs of each consumer/family • Possess organizational skills and the ability to multi-task • Must have outstanding communication and social skills • Ability to initiate tasks necessary for the program to run efficiently • Ability to work well under pressure • Ability to problem solve • High School Diploma or G.E.D. • Valid Arizona Driver’s License • Reliable means of transportation with current insurance and registration • Ability to obtain an Arizona Fingerprint Clearance Card • Ability to pass criminal, background and reference checks • Must pass Article Nine Training provided by Family Partners • Must pass Certification in Adult/Child/Infant CPR, and First Aid provided by Family Partners
