• Communicate with clients and their families to build rapport and understand the caregiver journey • Identify barriers to aging in place and reduce out-of-pocket expenses through phone and email support • Use motivational interviewing techniques to interview and engage individuals with differing levels of health needs • Document communication and intervention activity accurately and consistently • Serve as an expert in care management, caregiver journeys, care coordination, and resource management/navigation • Collaborate with the development team to improve care management tools and systems • Mentor new and junior team members as a care management subject-matter expert • Manage caseloads and meet outreach touchpoint goals at each engagement level • Complete needs assessments, outbound touchpoints within SLA, and accurate documentation reviews
• Strong background in and real-world geriatric case management or coordination experience required • Passion for helping older adults in all settings and situations • Comfortable communicating with clients using multiple methods, including phone and email • Professional and engaging phone presence • Creative problem-solving ability and comfort changing approaches • Comfortable with continuous change and product/program improvements • Willing to take the lead in developing and improving the care management program • Must be 18 years of age or older • Experience in Social Services, Care Coordination, Healthcare Navigation, Public Health, Community Organizing, or Volunteer Work highly preferred • 1 year of relevant customer service or similar experience preferred • High-volume call center experience is a plus • Bachelor degree in Psychology or Social Work preferred • Reliable internet connection and designated work environment conducive to professional phone calls and sensitive data • Ability to remain at a designated workstation for the duration of the workday • Ability to constantly operate a computer, phone, and VoIP equipment • Ability to communicate information and ideas clearly and exchange accurate information • Ability to observe details at close range, typically on a computer screen • Selected candidates may be required to undergo a background check and/or drug screen
