⢠Oversee day-to-day execution of programs, including intake, eligibility, service delivery, and follow-up ⢠Refine and maintain program policies, procedures, workflows, and documented SOPs ⢠Identify operational risks, access gaps, and awareness gaps; implement solutions to improve efficiency and participant experience ⢠Evaluate, select, and implement a long-term technology platform to replace manual and stopgap intake workflows ⢠Support development and management of program reports and budgets for leadership and funders ⢠Track, analyze, and report program outputs and outcomes ⢠Transition and own full Community Partner Program administration, including grant funding outreach, partner agreement renewals, report collection, tracking, and payment processing ⢠Serve as primary point of contact for Community Partners and manage partner relationships ⢠Build and maintain a targeted referral network of healthcare providers, imaging centers, social service agencies, and community organizations ⢠Lead outreach meetings and presentations with referral partners and support awareness and resource utilization ⢠Serve as a point of contact for complex or high-priority referrals ⢠Support grant applications with program data, narratives, and insights ⢠Maintain accurate and organized program documentation ⢠Supervise the Program Coordinator and provide guidance, escalation support, quality oversight, and professional development ⢠Partner with the Director of Operations, Development, and marketing partners on administrative, fundraising, reporting, and referral-tool needs
⢠Bachelor's degree in public health, social work, nonprofit management, or a related field, or equivalent professional experience ⢠Minimum of 5 years of experience in program coordination or management within nonprofit, healthcare, or human services on a national level ⢠Experience with grant-making and grant-distribution program model ⢠Minimum of 3 years of Supervisory experience ⢠Experience in medical advocacy, patient navigation, or public health ⢠Demonstrated ability to evaluate and implement technology or process improvements in a program setting ⢠Strong organizational, analytical, and project management skills ⢠Self-starter who thrives in a small, fast-paced environment ⢠Excellent written and verbal communication skills ⢠Comfort working in a remote team environment ⢠Must work EST hours 9am-5pm ⢠Spanish proficiency preferred ⢠PMP certification or equivalent project management credential preferred ⢠Familiarity with referral-based or patient/client service models preferred ⢠Experience with community outreach and referral network development preferred ⢠Familiarity with grant-funded programs and reporting requirements preferred
