📋 Description

• Conduct comprehensive virtual patient assessments, including medical and social history, visual and limited physical examinations • Apply clinical decision-making based on disease pathophysiology and pharmacology • Manage acute conditions of post-acute patients, including new-admission stabilizations • Conduct virtual patient assessments, history taking, physical examinations, and diagnostic evaluations • Diagnose and treat acute and chronic medical conditions • Prescribe medications and develop treatment plans • Collaborate with healthcare professionals to ensure continuity of care • Use telehealth technology to deliver high-quality care • Maintain accurate and up-to-date patient records • Adhere to clinical guidelines, protocols, and regulatory requirements • Participate in educational programs to increase clinical knowledge and skills • Demonstrate accountability for professional practices • Work Saturdays between 12:00pm CT and 9:00pm CT, with shifts potentially covering only part of that period

🎯 Requirements

• Master’s or Doctoral degree in Nursing from an accredited program • Current Nurse Practitioner (NP) license free of disciplinary action; approved specialties include FNP, ACNP, ANP, and AGNP • Current AANP or ANCC Certification • Current CRNP License • Current NCCPA Certification • Current BLS Certification • Minimum of 5 years of clinical experience as a Nurse Practitioner • Minimum of 3 years of advanced practice provider experience within post-acute care/skilled nursing care preferred • Experience in telehealth or remote patient care preferred • Excellent communication and interpersonal skills • Strong attention to detail • Proficiency with telehealth technology and electronic health records (EHR) systems • High level of general proficiency with computers and Microsoft Office • Ability to work independently and as part of a multidisciplinary team • Compassionate and patient-centered approach to care • Availability for occasional weekend and/or evening work • Ability to perform essential job functions consistently and successfully in accordance with ADA, FMLA, and other federal, state, and local standards • Ability to meet productivity standards • Ability to maintain the required physical and visual capabilities • HIPAA-compliant dedicated home office space with privacy for PHI/PII and a professional video-conferencing backdrop • Access to Wi-Fi
