📋 Description

• Manage ongoing care for assigned individuals to support continuity and stability across services and settings • Develop and maintain comprehensive care plans addressing medical, behavioral, and social needs • Coordinate services across providers and community resources • Monitor progress toward care goals and adjust care plans based on changing needs • Engage individuals and families to support sustained participation in services and care plans • Provide comprehensive care coordination for individuals transitioning between care settings and maintaining stability over time

🎯 Requirements

• Must meet QP status through one of the stated qualification pathways: fully licensed by the NC governing board regulating Human Services professions; or fully licensed RN licensed to practice in North Carolina with four years of full-time MH/DD/SA experience; or a master's degree in a human service field with one year of full-time MH/DD/SA experience; or a bachelor's degree in a human service field with two years of full-time MH/DD/SA experience; or a bachelor's degree in another field with four years of full-time MH/DD/SA experience • Substance abuse professional alternative pathways require the stated supervised alcoholism and drug abuse counseling experience • If serving members with LTSS needs, minimum two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience; may be concurrent with QP experience • Must have a valid driver's license • Must reside in North Carolina and within 75 miles of Trillium's Southern Region • Eligible counties: Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, and Scotland • Must be able to travel within catchment as required • All candidates must pass a drug test as a condition of employment • Preferred: Degree in Human Services
