• Provide in-reach and transition care management to members in institutional, congregate, or facility-based settings, including hospitals and inpatient units • Educate members, families, and guardians about integrated community-based care and available services • Facilitate access to physical, behavioral, social, and community resources • Provide complex care management in compliance with NCQA and required accreditation standards • Develop whole-person care plans using national evidence-based and evidence-informed standards • Provide guidance and monitoring to providers regarding social determinant, medical, and other member needs • Identify, document, and attempt to address barriers to relocation to integrated community settings • Engage members in State Psychiatric Hospitals and ACHs regarding permanent supportive housing and community resources • Complete required documentation, paperwork, and software-platform tasks within designated timeframes • Work on an interdisciplinary team addressing Olmstead Settlement and Tailored Plan functions • Assess and monitor member health and safety, including during emergencies or weather advisories • Travel within the catchment as required
• Must meet QP status • Must hold a qualifying full license from the NC governing board regulating Human Services professions, or be a fully licensed RN licensed to practice in North Carolina • Licensed RN pathway requires four years of full-time experience with the MH/DD/SA population • Master’s degree in a human service field plus one year of full-time MH/DD/SA experience, or one year of supervised alcoholism and drug abuse counseling experience • Bachelor’s degree in a human service field plus two years of full-time MH/DD/SA experience, or two years of supervised alcoholism and drug abuse counseling experience • Bachelor’s degree in another field plus four years of full-time MH/DD/SA experience, or four years of supervised alcoholism and drug abuse counseling experience • Must have a valid driver’s license • Must reside within 75 miles of Trillium’s Southern Region, specifically Lenoir County • Must be able to travel within the catchment as required • Must pass a drug test as a condition of employment • Resume must include employer names, dates of service, average weekly hours, essential duties, education details, and licensure/certification information if applicable
