• Support individuals transitioning between care settings by providing care coordination and transition planning services • Coordinate and manage care transition activities across settings to support successful discharges and service changes • Develop and implement transition plans based on individual needs and transition requirements • Collaborate with providers and partners to align services during transition periods • Monitor transitions and intervene to resolve delays or barriers • Facilitate service connections and ensure continuity following transitions
• Bachelor's degree in a human services field or licensure as a RN and three (3) years of relevant experience working directly with individuals with Serious Emotional Disturbance (SED) or Serious Mental Illness (SMI); OR Master's degree in a human services field or licensure as a RN and one (1) year of relevant experience working directly with individuals with Serious Emotional Disturbance (SED) or Serious Mental Illness (SMI) • Must have a valid driver’s license • Must reside in North Carolina and within 75 miles of Pitt, Lenoir, or Wayne County • Must be able to travel within catchment as required • This position has no licensure or certification requirements unless qualifying as a Registered Nurse (RN) • Experience and knowledge of peer support and experience working with individuals who are certified peer support specialists (Preferred) • All candidates must pass a drug test as a condition of employment
