📋 Description

• Engage members who meet criteria for TCL special needs population and provide education about permanent supportive housing, supports, services, and community resources • Partner with Diversion staff, Transition Coordinators, and Next Step Rehousing Team to facilitate transition planning and progression through the TCL process • Collaborate with Trillium Integrated Care Nurse, OT, COTA, and Clinician to identify complex care needs and clinical service linkage needs • Facilitate community integration, link members to services, and provide education and support for members who choose not to transition to permanent supportive housing • Use person-centered planning methods and strategies to gather information and address individual needs • Provide education and support to members and guardians regarding rights and responsibilities

🎯 Requirements

• High School Diploma or GED • One year of lived experience with a behavioral health condition and/or substance use • North Carolina Peer Support Specialist Certification • Must have a valid driver’s license • Must reside within Trillium’s Northern or North Central Region: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, Warren, Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrell, Washington, or Wilson counties • Must be able to travel within catchment as required • Must pass a drug test as a condition of employment • Resume must include employer names, dates of service, average hours worked per week, essential job duties, education details, and licensure/certification information if applicable
