📋 Description

• Provide care coordination, including treatment planning, assessment, monitoring, education, and referral/linking • Support individuals identified in the Transitions to Community Living Initiative Special Needs Populations • Provide care planning and discharge/transition planning from Adult Care Homes, inpatient units, and detention centers • Work collaboratively with licensed Clinicians, Registered Nurses, and Peer Support Specialists • Provide care coordination/care management to assigned TCL individuals with behavioral health, physical health, co-occurring, co-morbid, or multi-morbid needs • Assess members transitioning into supportive housing and determine additional support needed for continued independent living • Complete assessments as needed • Document planning and coordination efforts accurately and timely across multiple software platforms and State database systems

🎯 Requirements

• Must meet QP status through one of the listed licensure, nursing, or education-and-experience pathways • Fully licensed by the NC governing board regulating Human Services professions; or fully licensed RN licensed to practice in North Carolina by the NC Board of Nursing • RN pathway requires four years of full-time MH/DD/SA population experience • Master's degree in a human service field plus one year of full-time MH/DD/SA experience, or one year of supervised alcoholism and drug abuse counseling experience • Bachelor's degree in a human service field plus two years of full-time MH/DD/SA experience, or two years of supervised alcoholism and drug abuse counseling experience • Bachelor's degree in another field plus four years of full-time MH/DD/SA experience, or four years of supervised alcoholism and drug abuse counseling experience • Must have a valid driver's license • Must reside in Trillium's Mid State Region: Anson, Guilford, Montgomery, Randolph, or Richmond counties • Must be able to travel within the catchment as required • Must pass a drug test as a condition of employment • Preferred: Experience and knowledge of peer support and experience working with individuals who are certified peer support specialists
