📋 Description

• Provide paraprofessional home health and hospice services to patients in their homes following physician orders • Follow care plans developed by the RN and report client condition changes to the RN • Assist with personal care, including bathing, mouth care, denture care, skin care, grooming, toileting, transferring, ambulation, and dressing • Assist with household tasks, meal planning and preparation, feeding, light housekeeping, changing linens, bed-making, and laundering • Measure and record temperatures, pulse, respiration, and blood pressure as ordered • Perform range-of-motion exercises and other simple procedures as an extension of assigned therapy services • Assist clients with self-administration of medication when assigned by an RN and documented in the clinical record • Maintain professional relationships with clients and their families while working with minimal direct supervision • Maintain skills and follow agency policies, procedures, compliance requirements, core competencies, and code of ethics

🎯 Requirements

• High School diploma or GED required • Completion of a federally compliant training program as required by law/regulation • Successful completion of a competency evaluation program • CNA license for CT required • One year’s employment as an aide in an institution, home care, or hospice setting • Meet agency hiring criteria, including provision of professional references • Ability to function with minimal direct supervision • Ability to maintain a professional relationship with agency clients and their families • Responsibility for maintaining skills and adhering to agency policies and procedures on an ongoing basis • Ability to demonstrate Organizational Core Competencies and Organizational Code of Ethics and adhere to the Compliance Program • Current driver’s license • Reliable transportation to and from work site
