📋 Description

• Reviews, prepares and dispenses medication orders per physician order or prescription according to established policies, procedures, and protocols. • Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures and protocols. • Provides clinical consultation and clarification to practitioners. • Utilizes data to proactively identify patients who may benefit from population health program intervention; coordinates with Providers, care management, or other care team members to develop, document and execute drug therapy plans.

🎯 Requirements

• Five year bachelor of science degree in pharmacy from an accredited college of pharmacy. • Current registered pharmacist license and controlled substance license issued by the Michigan State Board of Pharmacy. • Basic Cardiac Life Support (BCLS) certification may be required (at discretion of manager). • One year of experience in a hospital or retail pharmacy preferred. • Knowledge of drug regimen review, pharmacy software for order entry, allergy screening, drug interactions, and other related topics.
