📋 Description
• Provide patient-centered MTM services to optimize medication therapy • Deliver Comprehensive Medication Reviews, Disease Specific Medication Reviews, and Targeted Interventions • Communicate and document clinical recommendations to prescribers
🎯 Requirements
• Minimum 1 year of experience in a clinical pharmacy role • Bachelor’s Degree in Pharmacy or PharmD from an ACPE accredited college or graduate of a foreign college of pharmacy • Current pharmacist licensure • Ability to acquire pharmacist licensure in multiple states if requested
