📋 Description

• Provide comprehensive pharmaceutical care by reviewing and optimizing medication orders • Monitor therapeutic drug regimens and participate in patient care rounds • Ensure safe, effective, and cost-conscious medication therapy • Collaborate with physicians, nurses, providers, and interdisciplinary teams • Provide medication education to patients, caregivers, staff, and providers • Precept pharmacy learners and support staff development and professional enrichment • Coordinate medication reconciliation, clinical interventions, and pharmacokinetic monitoring • Compound, dispense, and supervise medication preparation • Maintain inventory and controlled-substance and medication records • Participate in clinical research, quality improvement, patient safety, and cost management initiatives • Maintain adherence to regulatory standards and organizational values • Perform other duties as assigned

🎯 Requirements

• Bachelor's Degree or PharmD Degree from an ACPE accredited school of Pharmacy • 0-2 years of experience in pharmacy operations, or a relevant area • Knowledge of TJC, USP 797, 795 and 800, and federal and state regulations • Active Licensed Pharmacist in applicable states • Basic Life Support (BLS) within 90 days of hire • Advanced Cardiac Life Support (ACLS) within 90 days of hire • Ohio Pharmacist license obtained within 90 days of hire if applicable for work location • All experience must be paid and in the same related field • Part-time and PRN experience will be prorated based on hours worked per week • Volunteer work and internships for academic credit are not counted
