• Perform outbound patient calls regarding clinical programs, medication adherence, and closing gaps in care for value-based contracted patients • Call patients to ensure timely refills of chronic disease medications for diabetes, blood pressure, and cholesterol • Serve as liaison between patients, clinics, and pharmacies to minimize gaps between refills • Interpret patient medical history and communicate providers’ plans to patients • Navigate multiple Electronic Medical Record platforms • Document patient care activities accurately in electronic records • Perform medication reconciliation • Request medication refills according to clinic policy and protocol • Assist patients with medication access barriers such as cost • Identify patients requiring referrals to pharmacists, pharmacy medication programs, care management, clinicians, or Village at Home • Follow up with patients, providers, and pharmacies to ensure medications are picked up and taken as directed • Coordinate quality health maintenance before and after primary care visits • Schedule appointments and testing and order screenings under standardized protocols • Maintain current knowledge of quality measure requirements • Schedule patient appointments for refills and barriers • Assist other quality departments as needed • Coordinate with pharmacy staff, primary care office staff, and providers regarding referrals, orders, and tests • Contribute to improving clinic quality ratings through medication adherence • Identify opportunities for continuous process improvement
• High school diploma or GED equivalent required • Certified Medical Assistant, Certified Pharmacy Technician, or equivalent experience in a healthcare setting required • Intermediate to advanced keyboarding skills, including at least 35 WPM, touch typing, and document formatting • Secure home workspace with desk and chair and minimum background noise • Candidate-supplied home internet access • Minimum internet download speed of 100 Mbps and upload speed of 10 Mbps • Hard-wired internet connection using an ethernet cord; Wi-Fi cannot be used • 1+ years of healthcare setting experience preferred • Prior Electronic Health Record experience preferred • Previous call center or telephonic experience preferred • Experience with Medicare Advantage plans, insurance, and medication adherence preferred • Bilingual English/Spanish preferred • Certified Medical Assistant or Certified Pharmacy Technician certification, or equivalent healthcare-setting experience
