• Conduct outreach to newly referred clients and log activity in VyncaOS • Conduct 8–12 intake visits daily, ensuring efficient and accurate assessments • Assist clients with initial intake visits, including psychosocial assessments and care plans with goals of care • Review medical charts and document client medical needs and suggested interventions • Communicate critical information and client preferences to the care team for personalized care planning • Utilize evidence-based practices including Motivational Interviewing, Harm Reduction, and Trauma-Informed Care • Attend weekly Team Operations Meetings • Maintain an organized schedule to optimize client intake and engagement • Perform proactive outreach to no-shows or previously uncontacted clients when unscheduled • Remain available for warm transfers from field teams and provide same-day responses • Meet key performance metrics in a fast-paced environment
• Active, unrestricted California Licensed Vocational Nurse (LVN) license, or Bachelor’s degree in a related field, or Medical Assistant certificate issued by a recognized group (AAH, AAMA, AMT, NCCT, or NHA) • Minimum of 2 years of previous intake or admissions experience in a high call volume environment • Experience working with complex care clients and ECM Populations of Focus • Experience with Health Gorilla or Health Record Systems • Strong oral and written communication skills • Positive interpersonal skills with a collaborative attitude • Expert computer skills and proficiency in Google Workspace, MS Office, and internet tools • Ability to work independently and stay highly motivated • Capable of managing multiple tasks and navigating various platforms, adapting and shifting priorities as needed • Bilingual (English/Spanish) highly preferred • Willing and able to work Monday-Friday 8:30am-5pm Pacific Time, with the possibility of evenings and weekends • Applicants must be located in Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, Utah, or Washington • Background check, potentially including a drug test or other health screenings, required prior to employment • Employees in patient, client, or customer-facing roles must be vaccinated against influenza • Must complete identity and work eligibility verification using E-Verify upon hire • Occasional travel may be required • Required physical abilities include regularly sitting, standing, walking, talking, listening, observing, computer work, and use of laptop/video-conferencing technology; occasionally reaching, stooping, bending, kneeling, and lifting items weighing 25 pounds or less
