📋 Description

• Serve as the initial point of contact for patients entering the health system • Triage new patient appointments and assure timely scheduling • Review outside medical records, test results, and documentation for initial visits • Assess barriers to care and facilitate referrals to support services • Educate patients and families about diagnosis, treatment options, side effects, wellness, survivorship, and prevention • Coordinate care with multidisciplinary teams, physicians, caregivers, and referring providers • Track patients throughout the care continuum and identify bottlenecks or gaps in care • Support clinical trial eligibility assessment and referrals • Provide psychosocial screening and referrals to social workers, financial counselors, and support services • Facilitate shared decision-making and advance care planning • Attend Tumor Board Conference and coordinate post-conference care • Maintain communication with the tumor registry • Document patient encounters and services in EPIC • Develop patient education materials and community cancer-support relationships • Contribute to nurse navigator program development and performance improvement • Orient new staff and attend required meetings and training

🎯 Requirements

• Graduate from an Accredited School of Nursing • BSN required • Valid State of Florida Registered Nurse (RN) license • BLS certification required • Minimum 2 years of relevant work experience • Must comply with HIPPA, ACHA, JCAHO, OSHA standards and regulations • Must complete mandatory in-services and employee health procedures
