📋 Description

• Provide support to patients and health care providers regarding Patient Support services, specific diseases, and therapies • Assist patients and caregivers with assessments related to Patient Support services • Liaise between patients, families, and health care providers regarding medication access • Address inquiries about program services, disease states, and products • Provide ongoing support to promote patient adherence • Refer patients to health care providers for clinical questions beyond program materials • Document patient information and interactions in electronic patient records • Review and update files throughout the patient journey • Follow case-management protocols, including data collection, follow-up, therapy disruption/discontinuation monitoring, adverse-event reporting, and liaison with primary health care providers • Collaborate with reimbursement specialists and physicians to secure therapy funding and explore alternatives • Coordinate and track patient services according to KPIs to expedite therapy access • Participate in specialized therapy training • Attend pharmaceutical and patient meetings, conferences, and work with patient associations and advocacy groups • Coordinate with health care providers, pharmacies, and infusion/injection clinics regarding treatment logistics and issue resolution • Communicate with the manager, team, and client regarding patient-case management • Assist with training new Nurse Case Managers and provide feedback on protocols, databases, and processes • Report adverse events and severe adverse events according to program procedures • Perform other assigned duties and tasks, including possible standby/on-call work

🎯 Requirements

• Diploma in health care or a related field; bachelor’s degree preferred • Licensed Nurse (RN or RPN/LPN) with active registration from the College of Nurses in the province of employment is required • Ability to solve problems and work both independently and as part of a team • Strong analytical and problem-solving skills • Ability to work effectively across different functions and within a small team • Excellent teamwork skills and willingness to provide feedback • Ability to consistently deliver high-quality professional work in a fast-paced environment • Strong team leadership skills, including leading, managing, motivating, and training a high-performing team • Experience delegating and assigning tasks/work within a team • Ability to think logically, clearly, and conceptually, while demonstrating creativity and innovation • May be required to be on standby/on call
